You can submit your claim on-line as soon as you have applied.

Some forms can also be downloaded from the links on the right, printed out and returned to the address on them. They are in pdf format. To view and print PDF files, you must have the free Adobe Acrobat Reader installed.  All forms are also available from Marmion House, Lichfield Street, Tamworth, Staffs, B79 7BZ.
